SMS Terms & Conditions

Precision Pain Care Center offers an SMS/MMS text messaging program to assist patients and prospective patients with appointment-related communications.
PROGRAM DESCRIPTION
If you affirmatively opt in to receive text messages from Precision Pain Care Center, we may send you messages relating to:
• Appointment scheduling and rescheduling
• Appointment confirmations and reminders
• Initial consultation scheduling
• Follow-up regarding appointment requests
• Responses to questions or requests you send to our office
• Other non-promotional communications related to your appointments or interactions with our office
This is not a marketing or promotional text messaging program. We do not use this program to send advertising or promotional text messages.
CONSENT TO RECEIVE TEXT MESSAGES
Participation in the text messaging program is voluntary.
By providing your mobile telephone number and affirmatively opting in to receive text messages from Precision Pain Care Center, you consent to receive SMS/MMS messages at the mobile number you provide.
Consent to receive text messages is not a condition of receiving medical care, treatment, products, or services from Precision Pain Care Center.
If you choose not to receive text messages, we may communicate with you through other available methods, such as telephone or mail.
MESSAGE FREQUENCY
Message frequency varies depending on your appointments, scheduling activity, requests, and interactions with our office.
MESSAGE AND DATA RATES
Message and data rates may apply. Precision Pain Care Center does not separately charge you for participating in the text messaging program, but your wireless carrier’s standard messaging and data rates may apply.

HOW TO OPT OUT
Reply STOP to any text message from Precision Pain Care Center to stop receiving text messages.
After you reply STOP, you may receive one final confirmation message indicating that you have been unsubscribed.
Opting out of text messaging will not affect your ability to receive care or services from Precision Pain Care Center.
If you later wish to resume receiving text messages, contact our office at (626) 655-8286 and complete the applicable opt-in process.
CARRIER AND DELIVERY DISCLAIMER
Text message delivery is subject to your wireless carrier and network availability.
Carriers are not liable for delayed or undelivered messages.
Precision Pain Care Center cannot guarantee that messages will be delivered or received within a particular period of time.
IMPORTANT LIMITATIONS
Text messaging is not for medical emergencies.
Do not use text messaging to report a medical emergency, potentially life-threatening condition, or urgent change in your medical condition.
If you are experiencing a medical emergency, call 911 or seek immediate emergency medical attention.
Our text messaging service is not monitored continuously. Messages sent outside normal business hours, on weekends, or on holidays may not be reviewed until the next business day.
Standard SMS/MMS messaging may not provide the same confidentiality or security protections as other methods of communication. Please avoid sending highly sensitive medical, financial, or personal information by text unless specifically instructed by our office.
Please notify us promptly if your mobile telephone number changes or is reassigned.
PRIVACY
Information collected and used in connection with our text messaging program is handled in accordance with our Privacy Policy.
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes.
Text messaging originator opt-in data and consent will not be shared with any third parties.

We may update these SMS Terms & Conditions from time to time. The Effective Date at the top of this page indicates when these Terms were last revised.
